A Notice of Intent for the following project submitted in accordance with R307-401-1, Utah
Administrative Code (UAC), has been received for consideration by the Director:
Company Name: Ash Grove Cement Company
Location: Ash Grove Cement Company – Tooele Pozzolans Processing Plant – 1345 K Avenue, Tooele, UT
Project Description: Ash Grove Cement Company (Ash Grove) owns and operates a volcanic ash (pozzolans) processing facility. The facility receives wet ash product, where it is dried, processed, and sorted to the proper size for resale. The facility will produce up to 384,200 tons of material per year. The source has requested the following modifications to their Approval Order: Change ownership from Geofortis Utah, LLC to Ash Grove Cement Company, add one (1) storage silo, add one (1) loadout spout, remove the requirement to use dust collectors for processes involving saturated materials, add a backup feed conveyor to the dryer, and remove the 50-kW emergency generator engine from the permitted equipment. The completed engineering evaluation and air quality impact analysis showed the proposed project meets the requirements of federal air quality regulations and the State air quality rules. The Director intends to issue an Approval Order pending a 30-day public comment period. The project proposal, estimate of the effect on local air quality, and draft Approval Order are available for public inspection and comment at the Utah Division of Air Quality, 195 North 1950 West, Salt Lake City, UT 84116. Written comments received by the Division at this same address on or before December 20, 2024, will be considered in making the final decision on the approval/disapproval of the proposed project. If anyone so requests to the Director in writing within 15 days of publication of this notice, a hearing will be held in accordance with R307-401-7, UAC. Under Section 19-1-301.5, a person who wishes to challenge a Permit Order may only raise an issue or argument during an adjudicatory proceeding that was raised during the public comment period and was supported with sufficient information or documentation to enable the Director to fully consider the substance and significance of the issue.

BEEHIVE TELEPHONE PUBLIC NOTICE
Lifeline service is a government program designed to make monthly residential telecommunication services more affordable to eligible low-income customers. Customers who are eligible for the Lifeline program are also eligible for toll blocking at no additional charge.
The Federal and State Lifeline discount can be applied to qualifying voice or broadband services. The Lifeline program is administered by the Universal Service Administration Company for all lifeline services nationwide. To find out of more about the Lifeline program and eligibility requirements visit www.lifelinesupport.org/.
Lifeline is a non-transferable service and eligible subscribers may receive assistance from only one wireline or wireless telecommunications provider per household. Only eligible customers may enroll in the program. To verify eligibility and apply for Lifeline service discounts, the required application must be completed and certified online at nationalverifier.servicenowservices.com/lifeline. Customers who willfully make false statements to obtain Lifeline benefits can be punished by fine or imprisonment or can be barred from the program.
Basic services are offered to all customers in Beehive Telephone’s service territories at the rates, terms, and conditions specified in Beehive’s tariff. Eligible subscribers verified and certified by the Universal Service Administration Company will receive the discount prescribed by Federal and State regulation.
